The Texas Children's Hospital Health Information Management Department is committed to providing optimum customer service to every patient, family and healthcare professional. The Health Information Management Department will maintain the confidentiality of all protected health information, ensure records are properly secured and appropriately disclose information.

Health Information Management is open from 8 am to 4:30 pm, Monday through Friday, excluding observed holidays. We have several convenient locations to meet your needs.

Request your medical records

To receive a copy of a medical record, you must complete a Release of Information form (English and Spanish).

  • Be very specific about the information you need to have released. Indicate dates of service, types of visits and what parts of the record you need.
  • Sign and date the authorization using your full legal signature. We only allow handwritten signatures, electronic signatures are not allowed.
  • Please be sure to include your telephone number so we can contact you, if we have questions regarding your request.
  • The request may take approximately 10-14 business days to process. For faster service and minimal fees, we recommend if you have Texas Children’s MyChart Portal, you choose to receive the medical records electronically through you MyChart portal.

Who Can Authorize the Release of Medical Records

Parents or legal guardians may obtain and/or authorize the release of protected health information from their child's medical record. Individuals over the age of 18 must authorize the release of their own information.

Fees

Records requested for continuity of care are provided at no charge.

Fees and charges may apply. Fees are determined by the Texas Health and Safety Code Ann. §241.154(b). Fees are subject to change.

Texas Children’s has contracted with ScanSTAT Technologies; they will provide you with an invoice summarizing the charges. Payments can be make online https://www.scanstat.com/. Please note we cannot accept an electronic request for medical records on ScanSTAT’s website. Send authorizations directly to Texas Children’s with only handwritten signatures. For questions and concerns regarding billing/invoice, please call 866-442-9026.

To request a radiologic image, please contact the radiology department at 832-822-5324.

To request a hospital bill or itemized statement, please contact the billing department at 832-824-2300.

Request changes to your health record

Patients or legally authorized representatives may request an amendment to a health record if they believe their record needs to corrected or revised, by completing a Request for Amendment form.

Request a legal name change

Patients or legally authorized representatives may submit a Legal Name Change form. The form must be accompanied by one of the following documents to verify legal name change:

  • Birth certificate
  • Final adoption decree
  • Marriage certificate
  • Court order
  • Copy of driver’s license or state-issued ID card

Request a release restriction or limited access

Patients or legally authorized representatives may request restricted access to your record or your child’s record, please complete the Restricted Access form (English / Spanish).

Request a copy of a birth or death certificate

Although your child was born at our facility, or a family member passed away at our facility, the Health Information Management Department does not have or provide copies of certificates. Family or legally authorized representatives can contact the Office of Texas Vital Statistics to obtain these certificates.
